How does a person get into arrears with Sars?

If SARS calculates that the tax you should pay in one year is higher than what your employer paid via your PAYE / or that you paid manually, then you will owe SARS the difference. You can get into arrears by owing SARS more tax or by not completing and submitting tax returns every year.
